A carrier that loses its operating authority can register a new DOT number and keep the same trucks, the same phone, and the same yard. Nothing on that new record looks wrong, because the record is new. That is a chameleon carrier, and no single record will tell you about it.
So CarrierOk checks five identifying attributes against every other entity in the dataset and tells you which ones turn up elsewhere. Each match is named and counted rather than folded into a score, because what you do about a shared yard is different from what you do about a shared EIN.
